RUBY GEM PLAN
Assistance for an individual that requires intermittent assistance with Activities of Daily Living (ADLs): grooming, bathing, dressing, toileting, and ambulation. Assistance with 3 showers per week will be provided.
SAPPHIRE GEM PLAN
Assistance for an individual that requires continual assistance with Activities of Daily Living (ADLs): eating, bathing, dressing, toileting, grooming, transferring and ambulation. Assistance with 5 or more showers per week will be provided.
EMERALD GEM PLAN
Assistance for an individual that requires total assistance with Activities of Daily Living (ADLs): eating, bathing, dressing, grooming, toileting/incontinence management, one person assistance with transferring and ambulation. Assistance with daily showers will be provided.
DIAMOND GEM PLAN
The plan includes all of the services provided in the Emerald Care Plan PLUS daily services for unmanaged incontinence and if needed two person assistance with transferring and ambulation.
Additional services available include: Escort Service, Incontinence Management and Additional Laundry.
